If you would like to make a stock donation please complete the following three steps:

*Indicates a required field. If you do not fill out this form your stock donations will be anonymous.

1. Complete the form and click on the SUBMIT button below:

2. Contact your broker or stock holding company. Inform them which stocks and how many shares you will donate to CAI as a tax deductible contribution.

3. Have your broker contact Tyson Hunter (CAI broker) to confirm your DTC stock transfer (ticker and number shares) to CAI.
